- You may download our application
from this website in PDF or MSW format, fill it out
and mail it to us. To request a paper application,
email info@wydevue.com
and write “Request Application” on the subject
line.
- In addition to the application,
the following items will need to be mailed to WydeVue,
LLC in order to complete the process of your application:
- Signed copy of the application
by all principles.
- Signed copy of Terms and Conditions
(which includes the Privacy Policy and Code of
Conduct), Waiver of Liability and Confidentiality
Disclosure. (Click here for pdf version of Waiver
of Liability and Confidentiality Disclosure.)
- Completed and signed w-9 Form.
(NOTE: in the event that WydeVue acts as the General
Contractor, we will need to have this on hand.)
- Application
Fee of $49 for Paper Version of the Application.
- Copies of your current Trades licenses
required by state, city and/or county.
- Copy of General Liability Insurance
(min. 500/100 coverage)
- Copy of Work Comp Insurance (required
if you have employees), or a waiver from the state
signed and approved by the state.
- Copy of Car insurance on the vehicle(s)
used for business. Requirements pg 1 of 2 (rev.4/07)
- You will have the option of
providing credit card information for the application
fee and membership cost, or you may include a check
with the documentation that will be mailed to WydeVue,
LLC
- Membership cost is based on
each county. Please click on county map to review
which county(s) you want to Market your business in.
- $10 per month. One year minimum membership is
required. Other payment options include the following:
— pay $100 for a full
year (a $20 savings)
— pay $30 quarterly
- Membership processing takes up to 10 business
days. (NOTE: All required paperwork, fees and
cost need to be complete and sent to WydeVue,
LLC before processing begins.)
